Engagement Specialist, Experienced
Job Description:
- Serve as the first point of contact for patients through onboarding, scheduling, and relationship-focused outreach
- Facilitate CHE completion in nephrology offices, clinics, or via telehealth
- Coordinate with Nurse Practitioners and clinics to prepare for in-person appointments
- Make outbound calls to patients, caregivers, and clinics for CHE scheduling, reminders, and rescheduling
- Schedule telehealth CHEs for patients unable to attend in person
- Welcome new patients and collect concerns from existing patients
- Escalate concerns to RNs or Nurse Practitioners as appropriate
- Maintain accurate documentation and communicate patient updates to the care team
- Build trust, influence patient participation, and overcome engagement barriers
- Organize outreach by urgency and importance to maximize patient impact
- Function across both ES and CC capacities
Requirements:
- High School Diploma or GED required
- Associate’s or Bachelor’s Degree preferred
- Prior IKC experience required (CC/ES)
- 2+ years professional work experience in a customer service, patient-facing, or telephonic outreach role preferred
- Prior experience in a healthcare setting preferred
- Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook
- Strong verbal and written communication skills
- Active listening and motivational techniques
- Highly organized, with excellent time management and follow-through skills
- Ability to work autonomously and collaboratively with care team members
- Must reside in Pacific or Mountain time zones
- Quiet, private home workspace with high-speed internet required
