Dedicated Care Coordinator – Secondment

Job Description:

  • Support patients’ access to PDPA and VFM products prescribed by licensed healthcare providers
  • Manage and progress the patient journey from HUB enrollment through post-fill follow-up
  • Own 100% of assigned cases across the designated geography
  • Serve as the primary contact for patients/caregivers and healthcare providers regarding reimbursement and access support
  • Conduct benefits investigations and verification, including calls with payers and patients
  • Support prior authorization submissions, verification, appeals, and related deadlines
  • Explain benefits, out-of-pocket costs, deductibles, copays, reimbursement programs, maximizers, and accumulators
  • Verify product and baseline-test coverage across PBM and major medical insurance for VFM
  • Coordinate patient appointments, baseline assessments, testing, and vendor communications
  • Administer Interim Care, extended Interim Care, vouchers, copay cards, and copay eligibility attestations
  • Triage voucher, paid prescription, and Interim Care fills within Polaris
  • Conduct post-prescription follow-up at 10 and 45 days
  • Manage case records, documentation, secure messages, texting, faxing, prescription triage, and patient consents
  • Coordinate with HUB partners, Pfizer colleagues, Patient Support Program/HUB employees, and third-party vendors
  • Report adverse events and follow company compliance, safety, confidentiality, and content-control requirements
  • Use approved materials to communicate access options and address patient access barriers
  • Build trust through compliant follow-up and responsible written and spoken communication

Requirements:

  • B.S. or B.A. Degree
  • 8+ years of pharmaceutical (or related) experience in customer-facing roles/functions
  • Experience as an FRM, PAC, ACM, FAS, or working experience within access and reimbursement
  • Demonstrated ability to communicate directly with patients, providers, and cross-functional colleagues
  • Proficiency conducting benefit investigation and verification directly with payers
  • Working knowledge of prior authorization and appeals processes
  • Working knowledge of copay programs, copay maximizers and accumulators, vouchers, and free or interim care programs
  • Ability to own a high volume of concurrent cases with disciplined documentation and follow-through
  • Ability to meet adverse event reporting obligations with frequent, direct patient contact
  • Strong understanding of reimbursement, government payment systems, specialty pharmacy networks, co-pay assistance programs, and specialty medications
  • Ability to communicate and collaborate compliantly with cross-functional field-based colleagues
  • Alignment with HQ stakeholders to support patient access
  • Flexibility to work across different time zones if needed
  • Ability to travel approximately 10% to 20%
  • Candidates must be authorized to be employed in the U.S. by any employer
  • Permanent work authorization in the United States is required
  • U.S. work visa sponsorship is not available now or in the future
  • Nice-to-have: patient/customer-facing access and reimbursement education experience
  • Nice-to-have: proficiency in Polaris, including case management, documentation, prescription triage, and interpretation of PA, appeal, and patient enrollment documents
  • Nice-to-have: HUB experience
  • Nice-to-have: experience with Kiteworks or comparable encrypted email, DocuSign, and copay portal administration
  • Nice-to-have: experience coordinating with third-party vendors on patient assessment, testing, or specialty pharmacy logistics
  • Nice-to-have: experience supporting Interim Care, free goods, or bridge programs
  • Nice-to-have: experience with pharmacy benefit and major medical benefit coverage pathways
